Requirements for Lifeline Phone/Tablet
Lifeline is a government program that subsidizes phone service for low-income consumers, ensuring everyone can stay in touch. The Lifeline program is only available to eligible consumers who can provide documentation demonstrating eligibility for the program.

We may not use all proofs however,
it is preferred for you to bring all proofs that everyone in household receives, to prevent you going back and forth for proofs. This Includes Medicaid/CHIP/Food Stamp Cards, Current SSI Award Letters, Unexpired Benefit Letters, Most Current Utility Bill, Last years W2 , Current Pay Stub
